Minimum Submittal Requirements

1. Site Plan (digital, legible, 300 DPI min. if scanned)
2. Utility Franchise Permitting Guide - read before submitting
3. Latest Utility Franchise Application with Dewatering Affidavit
4. Authorization Letter from Utility Company (UAO) must be on file or submitted.
   • This document must state that the individual signing is authorized to execute on behalf of the entity.
